Advantage Preschool and Child Care

This State of Michigan Form is extremely important!  It contains the parent's contact information, including other emergency contact names and numbers and to whom your child may be released. It also serves as a medical release should an emergency medical situation arise, and it lists all your child's pertinent medical and insurance information.

This is another State of Michigan required form, which acknowledges that you have received certain documents such as the Licensing Rules for Family and Group Child Care Homes, a copy of my discipline policy, and also outlines from you several health-related questions about your child.

This form is only available from me.

This form serves to give me some background information on your child. You can tell me about your child's likes and dislikes, habits, and prior learning experiences.

This is a form that is only signed once at the initial enrollment, which gives permission for Advantage Preschool to take your child off the premises to go to the community park and/or take a walk around the neighborhood. These do NOT involve transportation.

Advantage Preschool regularly applies sunscreen to children before extended periods of time outdoors. This is a permission slip to apply sunscreen to your child.

With this form you can accept or decline the use of your child's photo for use at Advantage Preschool.
